This activity session will focus on guidance and consulting on utilizing data from your population health or QI departments to organize your approach—may be by demographics (age, neighborhood, etc) or by clinical conditions (diabetics, PLWHA, homeless, migrant farmworkers) Attendees will leave the sessions with a plan to implement at their organization.
